Applies mean sac diameter and serum β-hCG discriminatory thresholds to distinguish a normal early intrauterine pregnancy from a non-diagnostic scan or a failed / ectopic pregnancy.
The 2013 Society of Radiologists in Ultrasound multispecialty consensus deliberately raised the diagnostic thresholds for a failed pregnancy to avoid inadvertent harm to a potentially normal early gestation. Discriminatory values describe the point at which a structure should be visible: a yolk sac by a mean sac diameter of about 8 mm, and an embryo with cardiac activity by a mean sac diameter of about 25 mm. A single β-hCG level should never be used in isolation to diagnose ectopic pregnancy.

🎯 When to Use
- First-trimester bleeding, pain or pregnancy of unknown location.
- An intrauterine sac with no embryo, to judge whether findings are diagnostic of failure.
- Correlating a serum β-hCG level with transvaginal findings.
⚙️ How It Works
- Measure the mean sac diameter as the average of three orthogonal internal sac dimensions.
- Record whether a yolk sac, embryo or cardiac activity is present.
- Compare against the SRU diagnostic thresholds; a yolk sac is expected by MSD ~8 mm and cardiac activity by MSD ~25 mm.
📊 Interpreting the Result
- MSD ≥25 mm with no embryo is diagnostic of a failed pregnancy (anembryonic).
- An embryo with crown-rump length ≥7 mm and no cardiac activity is diagnostic of a failed pregnancy.
- Empty sac with MSD <25 mm is non-diagnostic — a follow-up scan is required; do not diagnose failure prematurely.
- Discriminatory β-hCG for a TVUS gestational sac is ~3500 mIU/mL, but no intrauterine sac at this level suggests, though does not prove, an abnormal or ectopic pregnancy.
⚠️ Limitations & Pitfalls
- Multiple gestations elevate β-hCG, invalidating single-pregnancy discriminatory levels.
- MSD is an imprecise predictor of outcome and should not be used alone to diagnose failure below the diagnostic cut-off.
- A pseudogestational sac of ectopic pregnancy can mimic an early intrauterine sac; look for a yolk sac and eccentric implantation.
