Clinical presentation
Woman of reproductive age with a positive pregnancy test, pelvic pain and vaginal bleeding, and an empty uterus on ultrasound.
Key imaging findings
- Empty uterus with a positive beta-hCG above the discriminatory zone
- Adnexal tubal ring / bagel sign separate from the ovary
- Ring of fire — peripheral vascularity on colour Doppler
- Free fluid / haemoperitoneum in the pouch of Douglas
