Woman of reproductive age with a positive pregnancy test, pelvic pain and vaginal bleeding, and an empty uterus on ultrasound.

Key imaging findings

  • Empty uterus with a positive beta-hCG above the discriminatory zone
  • Adnexal tubal ring / bagel sign separate from the ovary
  • Ring of fire — peripheral vascularity on colour Doppler
  • Free fluid / haemoperitoneum in the pouch of Douglas
