Clinical presentation

Older adult with subacute headaches, new focal neurological deficit and a first seizure.

Key imaging findings

  • Heterogeneous mass with thick, irregular peripheral (rim) enhancement and central necrosis
  • Extensive surrounding non-enhancing T2/FLAIR signal (infiltrative oedema and tumour)
  • Crosses the corpus callosum — the butterfly glioma pattern
  • Restricted diffusion in hypercellular regions and elevated rCBV on perfusion
