Clinical presentation
Older adult with subacute headaches, new focal neurological deficit and a first seizure.
Key imaging findings
- Heterogeneous mass with thick, irregular peripheral (rim) enhancement and central necrosis
- Extensive surrounding non-enhancing T2/FLAIR signal (infiltrative oedema and tumour)
- Crosses the corpus callosum — the butterfly glioma pattern
- Restricted diffusion in hypercellular regions and elevated rCBV on perfusion
