Clinical presentation
Young woman with an incidental adnexal mass found during pelvic ultrasound for unrelated pelvic discomfort.
Key imaging findings
- Echogenic mural nodule (Rokitansky/dermoid plug) projecting into the cyst
- Dot-dash / lines-and-dots echoes from hair within the fluid
- Fat–fluid level and macroscopic fat (very low HU on CT, drop-out on fat-suppressed MRI)
- Acoustic shadowing from the tip of the iceberg — echogenic fat and calcification
