Clinical presentation

Elderly patient on anticoagulation with progressive confusion and headache weeks after a minor fall.

Key imaging findings

  • Crescentic (concave) extra-axial collection following the convexity
  • Crosses cranial sutures but not the midline (limited by the falx)
  • Density varies with age: hyperdense (acute), isodense (subacute), hypodense (chronic)
  • Acute-on-chronic collections show a fluid–fluid or layered appearance
