Clinical presentation
Elderly patient on anticoagulation with progressive confusion and headache weeks after a minor fall.
Key imaging findings
- Crescentic (concave) extra-axial collection following the convexity
- Crosses cranial sutures but not the midline (limited by the falx)
- Density varies with age: hyperdense (acute), isodense (subacute), hypodense (chronic)
- Acute-on-chronic collections show a fluid–fluid or layered appearance
